Foliotation moderne en bas des ff.: 1-161. Dans la foliotation ancienne, le f. 50 a été chiffré 2 fois, le 114 a été sauté, 2ff. n. ch. entre 94 et 95. Dans la foliotation moderne les ff. lr-v, 3, 53v, 54, 99v, 100, 156v, 157, 160v, 161r-v sont blancs. Notation mesurée blanche. 10 portées par p. Au f. 2r-v: «Jesus Maria. Tabula Motetti scripti Anno MQXXXVI.» Aux ff. 3v-4, cinq miniatures avec les armes de la Cappella Giulia et l'arbre de la famille Della Rovere, fondatrice de la chapelle. Rehure du XVIe s. cuir estampé, coins métal, portant au centre du plat antérieur l'écusson de la Cappella Giulia et l'inscription: «Cappella Nativitatis beate Marie in sancto Petro», au plat postérieur l'image de Saint Pierre, les armes de Mons.
